Payment Options

Forms of Payment You may choose from a variety of  payment options at Sundet Dental and Facial Aesthetics PLLC: Visa, MasterCard, American Express, personal check or cash.  Please make note that payments are due the day of service, unless otherwise arranged.

Dental Insurance Our office is considered a fee for service practice. We accept all forms of insurance. However, our office maintains no contractual agreement with any insurance company or third party payer except for Delta Dental Premier Plans. This means that we are considered an “out of network” provider. As a courtesy and to best assist our patients, we will always submit claims and prior authorizations when appropriate as well as appeal any claims on behalf of the patient when necessary. Patients are responsible for all balances not covered by their insurance. We will follow up and do whatever we can to help you access the benefits you deserve.
